Facility Excellence Nomination

Facility Excellence Award Criteria:

The Facility Environmental, Health and Safety Excellence Award honors a facility that has identified and evaluated environmental health and/or safety hazards in the workplace and demonstrated excellence through the implementation of new controls that significantly reduced risk.

Examples of award criteria the applicant companies may consider include the following:

  • The facility used a systematic approach to recognize and understand the scope of the hazard or environmental concern and determined a need for intervention (examples may including initiating a new audit program, or analysis of accident/incident statistics).
  • The facility involved employees from various departments (generally viewed as a facility wide effort).
  • The control approach was innovative or a generally accepted practice that controlled or eliminated risk in the workplace or the environment.
  • Demonstration or evidence of the progress was well documented in measured data.

Please provide the Facility/Plant Name and full physical address (Street Address, City, State/Province, Postal Code and Country)
Please describe the state or nature of the problem and the approach the facility used to identify the hazards or environmental concern. This field should describe the state of the facility BEFORE the initiative or intervention was implemented.

Describe how the approach was innovative if applicable. Describe the level of employee involvement across the facility, which departments were involved, and how much time and effort was needed for full implementation.
Include any data the facility used to measure success.
